The following procedures must be followed:

•  Request for verification of academic information, stating the following: the complete name (used while in school) of the student/graduate and the purpose of verification must be submitted using the company letterhead.
• The letter must be duly signed by the Human Resource Manager/Personnel Officer or its equivalent (for employment verification) and by the Head of office or its equivalent (for academic verification) of the government /non government agency.
• Attach a copy of the documents to be verified/authenticated.
• Attach the authorization to release records signed by the applicant.
• Verification of records through phone shall not be entertained. However, information like year of graduation and degree earned may be allowed.
